I agree to pay for all services rendered and understand that treatments are non-refundable once administered.

This consent form applies to therapies that may include peptide-based treatments or GLP-1 medications.

GLP-1 medications belong to a class of drugs known as GLP-1 receptor agonists used in weight management and metabolic health. Examples include medications such as Semaglutide and Tirzepatide.

Peptides are chains of amino acids that may influence biological functions such as metabolism, hormone signaling, and cellular repair.

Some peptides and metabolic therapies may be prescribed off-label by licensed medical providers.

Certain peptides and compounded medications may not be approved by the U.S. Food and Drug Administration for specific indications.

Possible side effects may include:

I authorize Z & D Health and Wellness to take photographs or videos before and after treatments for medical documentation purposes.

With my consent, these images may be used for:

• educational purposes
• marketing materials
• social media content
